I. SB 19 (WOLK) – POLST REGISTRY – SECTION 4788 – STATS. 2015, CH. 504
New section 4788 was added to the Probate Code to authorize the California Health and Human Services Agency (HSA) to establish and operate a statewide registry for the purpose of collecting POLST forms received from health care providers (or physician designees).
Health care providers who complete a POLST form are required to include the POLST form in the patient’s medical record and to submit the form to the registry, unless the patient or his or her health care decision maker chooses not to participate in the registry.
